Please note that all fields are mandatory. In order for us to process your request, we need to ensure we have all the information required.

This form is only to be filled out by players that have agreed their signings with their team managers.  It is not possible to join a team without arranging a trial and being invited to join by a team manager. 

If you require any other assistance, please contact your team manager.  This form is secure and only our office staff receive copies of this information just as you would hand us a piece of paper.  We also require an image of the players photo ID, this is required so that we can confirm the birthdate of the player, and we register that player with the league so we need to have an image of the player as well as proof of age.  At the bottom of this form you will be able to upload images from your device to include with this form.  The image should be similar to a passport with a clear head and shoulders on a blank background and can be taken with your phone.

Membership Fees

Subscription includes training and use of facilities at AFC Whyteleafe Youth, league insurance, league registration fees, affiliation fees.

The club will provide player insurance (via Surrey FA) training equipment/balls and first aid kits.

Club membership will provide access to fund raising activities.

Payment in full is preferred upon registration, however, 50% is acceptable on registration, with the balance being due at the end of August, before the season begins. AFC Whyteleafe Youth reserve the right to withhold the players registration until the fees are paid in full.

Any parent wanting to add a voluntary 10% to their fees in support of the first team, please let us know. The 1st team will provide a pathway and a great opportunity for our youth players to progress on to, so by support would be great fully received.

Subscription fees are non-refundable. Non-payment will result in the player being suspended until payment is made.

 

Health and Relevant Information

Does the player have any special health needs (e.g. Asthma, Epilepsy, Allergies, Diabetes etc) or special health provision (e.g. Epi-Pen, Inhaler etc) or any other relevant information (e.g. religious, cultural, dietary etc). Please specify or write ‘none’.

 

Medical Consent

I give consent that in an emergency situation the club may act in loco parentis, if the need arises for the admission of emergency first aid and/or other medical treatment, which in the opinion of a qualified medical practitioner may be necessary. I also understand that if such a situation occurs all reasonable steps will be taken to contact me or the alternative person named on this form. I confirm that to the best of my knowledge my child does not suffer any medical condition other than those detailed by me above.

By checking the above Medical Consent box and filling in my name I agree that all the information above is correct and my submission of this form confirms I fully understand the responsibilities required and undertake these responsibilities.

 

Data Protection

The club will use the information provided on this membership form to administer his/her football activity at the club and in any activities in which he/she participates through the club and to care for and supervise activities in which he/she is involved. In some cases, this may require the club to disclose the information to leagues and the FA. In the event of a medical issue or child protection issues arising, the club may disclose certain information to doctors or other medical specialists and/or to police, children’s social care, the courts and/or probation officers and potentially to legal and other advisers involved in an investigation. As the person completing this form you must ensure that each person whose information you include in this form knows what will happen to their information and how it may be disclosed.

At times the club may wish to take photos or videos of the team or individuals in it. We adhere to the FA guidelines to ensure these are safe and respectful and used solely for the purposes for which they are intended, which is promotion and celebration of the activities of the club and for training purposes. Player Image and ID Upload

In order to process the membership form we require a passport style photo of the player (clear head and shoulders with blank background) as well as proof of ID that we submit to the league. Player photos should be shoulders and head similar to a passport photo so that we can identify the player. Proof of ID can be a photo of a birth certificate or any other form of ID that shows the players birth date with their name.
